In my first run of the GY/Cath wing I was unable to enter the cathedral. The concept is to kill the monk outside and the doors open, which is exactly what happened. However there is a clipping issue where even though the doors have opened there was an "invisible wall" preventing me from entering the cathedral and finishing the instance.
