When I'm in a dungeon, the last thing I need is to hear "Take THIS!" spammed over and over by a Paladin who bound it with Templar's Verdict.
When I'm in a raid, I don't need the Rogue rambling on about how he's the 'master of the shadows' while I'm trying to do callouts.
When I'm in the world doing questing, inevitably a Mage will catch up with me saying "BY FIRE BE PURGED!" whenever he casts Pyroblast.
And in PvP, depending on who you ask, it can be considered toxic behavior. I personally don't think so, but along with that, it shares the same issue with raids.
If I wanted to listen to people LAMMORP, I'd join an RP server.
Please stop.
EDIT:
So, I've seen a lot of people with macros for out of combat things, such as rez, mage food, closet, etc. I really don't have a problem with those. It's when I'm actually trying to focus on a fight, or focus on getting an objective done, that it becomes an issue with me.
