Lava Surge is indeed currently bugged. We'll try to get it hotfixed on PTR so that it doesn't disrupt testing. Thanks for the report.

No, it was 100% on PTR due to a bug. It's been unchanged on live throughout Mists. Fixed on PTR now, though.
