The cleave effect from Heart Strike seems to be breaking auto-attack.
How to test
Go find 2 target dummies, relatively close together. Heart strike one of them. You'll see a white attack, the yellow heartstrike on your main target, and the heartstrike cleave on your secondary target. Now walk away from your keyboard and just watch.
You'll never see another auto-attack. Instead every ~3 seconds (depending on weapon swing speed) you'll see a red error message saying "You are not close enough."
So basically whats happening is that the auto-attack target is being switched to targets hit by hearstrike cleave and until you use a different ability other than hearstrike, or change targets, you won't get anymore auto-attack swings. This is unfortunate because many blood builds are built around Death Rune Mastery, where you use 2 obliterates/death strikes to get all your frost/unholy converted into Death Runes, then you have 6 Blood/Death runes to dump on consecutive Heart Strikes.
With that rotation, if there's a secondary target being hit, you will no longer see more than one auto-attack for one entire rune-refresh cycle since you will be using nothing but Heartstrike for 6 consecutive attacks. That's basically 10 seconds of no auto-attacking.
